    THE FORM 1095B - HEALTH CARE TAX FORMS FOR 2016 HAVE BEEN MAILED
    Starting for calendar year 2015, members of Local 94 will receive Form 1095-B from the Health and Benefit Fund of Local 94 indicating proof of health coverage . The IRS extended the reporting deadline (for 2016 only) to March 2, 2017 and has clarified that individuals do not have to delay filing their income tax returns if they have not yet received Form 1095-B. Also, individuals don’t have to file an amended return if they already filed their tax return before receiving Form 1095-B. If individuals file their income tax return AFTER receiving Form 1095-B, for 2016 they don’t have to include the form with the return; it should be kept with their records.     ***IMPORTANT REMINDER*** New Eligible Dependents
    You have 90 days to enroll all new eligible dependents (e.g., spouses and/or children) as of their applicable date (i.e., the date of marriage, the child’s birthdate, date of adoption or placement for adoption or foster care). If you fail to do so within the applicable 90 day period, dependent coverage will not be available under the Plan for your spouse and/or dependent child until the first day of the month following the date in which you provide the Fund Office with the required documentation and any other verifying information requested. Reminder: Union Dues
Please note, effective January 1, 2017, the maximum monthly dues are $87.55. Dues for 40 hour contracted work weeks are calculated by multiplying the hourly rate x 2.50; for 35 hour contracted work week hourly rate x $2.20; for 37 1/2 hour contracted work week hourly rate x $2.36, up to a maximum of $87.55. When the hourly rate increases, the dues increase, up to the maximum monthly dues.
